All potential new residents are required to complete a new member application package and submit it, along with all additional required documentation.  Complete application packages will be processed within 14 calendar days.

The package includes:

• Letter to Prospective Residents

• Application for Membership

• Consumer Authorization and Release Form (to be completed by all applicants 18 years or older)

• Community Rules/Bylaws Acknowledgement Form

• DMV Background Check Form

In addition to the completed forms, all applicants over the age of 18 must include the following items in when submitting their application package:

​

• Copy of a photo identification

• Non-refundable application fee - $100 (in the form of a check made payable to Lee Oak Cooperative, Inc. cash is NOT accepted) for each applicant 18 years an older

• Proof of income, including the previous four consecutive weeks pay stubs and the previous years Federal Income Tax Return, proof of Social Security and/or SSD income, annuities and/or proof of any other sources of income.
